Admission Application

護照

Passport

●無護照者，請上傳國籍身分證件。

If you do not have a passport, please upload your national identity document.

●申請人如有居留證，請務必一併上傳。

If the applicant has an Alien Resident Certificate, it must also be uploaded.

最高學歷或同等學力證明文件

Highest Academic Qualification or Equivalent Credential Certificate

●畢業證書如非中文或英文，需附上經公證之中文或英文譯本。

If the diploma is not printed in Chinese or English, an additional notarized Chinese or English translation must be provided.

●應屆畢業生請上傳在學證明。

Graduating students should upload proof of enrollment.

歷年成績單

Full Academic Transcript

●歷年成績單如非中文或英文，需附上經公證之中文或英文譯本。

If the academic transcripts are not in Chinese or English, an additional notarized Chinese or English translation must be provided.

財力證明

Proof of Funds

●財力證明，須於3個月內所開立。

A proof of funds issued within three (3) months.

●須由金融機構出具本人足以支應在臺就學花費之財力證明，帳戶餘額須達美金3,500元以上。

A proof of funds issued by a financial institution proving that the applicant has sufficient funds to cover their studies in Taiwan, with an account balance of at least US$3,500.

●若申請者本人無銀行帳戶，本校僅接受申請人以法定親屬帳戶開立之財力證明替代之。申請者需另外檢附與資助人之親屬證明並由資助人出具資助同意書，若僅提供資助同意書，將視同未檢附財力證明。

If the applicant does not have a bank account, CGUST will only accept a proof of funds from an account opened by a legal relative instead. The applicant must also provide proof of kinship with the sponsor and a letter of consent from the sponsor. Providing only a letter of consent by the sponsor will not be considered a proof of funds.

●若提供政府、本校或民間機構之獎助學金證明，所獲獎學金總額需達美金3,500 元)。申請中之各類獎學金申請表不能作為財力證明。

If a proof of scholarship or grant from the government, CGUST, or a private organization is provided, the total amount received must total US$3,500). Pending scholarship application forms cannot be used as proof of funds.

●財力證明不得採用代辦、公司行號或個人開立證明。

Financial proof must not be issued by agents, companies, or individuals.

語言能力證明

Proof of Language Proficiency

●檢附文件－語言能力證明：
申請者須依報考系所規定檢附下列語言能力證明文件之一或數項：

Required documents - Language proficiency certificate:
Applicants must submit one or more of the following proofs of language proficiency as required by the department of application:

1. 華語文能力證明（TOCFL）基礎級（A2）以上（適用於部分系所，為必備文件）

Test of Chinese as a Foreign Language (TOCFL) Basic Level (A2) or above (a required document for certain departments)

2. 英文能力證明（CEFR B1（含）以上）（適用於部分系所，為必備文件）

Proof of English proficiency (CEFR B1 or above) (a required document for certain departments)

各系所之語言能力必備項目，請參閱「學系特色及審查項目」。

For each department's language proficiency requirements, please refer to Department Features and Review Items.

●如符合以下條件，可提供相關證明，不須再繳交語言測驗證明。

If you meet the following conditions, you can provide relevant proof and do not need to submit a language test certificate.

1. 國籍為英語系國家，請提供護照或國籍身分證件作為證明。

Those who hold the nationality of an English-speaking country should provide their passport or nationality identity document as proof.

2. 先前任一學位(含高中畢業證書)在英語系國家取得，請提供您的畢業證書作為證明。

Those whose previous degree (includes high school diploma ) was obtained in an English-speaking country should provide the diploma as proof.

3. 先前任一學位(含高中畢業證書)為全英語授課，請提供學校開具的證明文件。

Those whose previous degree (includes high school diploma) was taught entirely in English should provide supporting documentation from the school.

學習研究計畫書

Statement of Purpose

●請以英文撰寫。

It should be written in English.

●內容可包括選擇本校或系所的動機以及就讀本校時的短、中、長程學習計畫。

The content may include the motivation for choosing CGUST or the department, as well as short-, medium-, and long-term study plans while studying at CGUST.

自傳

Autobiography

●請以英文撰寫。

It should be written in English.

系所規定之其他應繳文件。

●請從申請系統下載範例並列印填寫及親筆簽名，完成填寫後上傳檔案。

Please download the template from the application system, print it, fill it out, and sign it in your own handwriting. After completing the form, upload the file.
